A fire at New York's Triangle factory killed 146 people in 1911. Workers had needed to escape through rooms crowded with sewing machines.
The state investigation later called for clear paths from work areas to exits. Having doors was not enough if workers could not reach them.
The space between the machines mattered too: it was part of the route people needed to take to get out.
